Application requirements for an
individual. (1) Applications for an individual shall be
submitted on forms prescribed by the department and shall be
accompanied, as appropriate, by either:
(a) Documentation that the applicant has met the required
training, experience, and education requirements as described
in WAC 365-230-132, 365-230-134, and 365-230-380. Acceptable
documentation includes the following:
(i) As proof of meeting the training requirements, a
valid lead-based paint training course completion certificate
issued by a department-accredited training provider.
(ii) As proof of meeting the work experience
requirements, documentation must include name and address of
employer, name and telephone number of supervisor; employment
dates, description of specific duties performed. The
supervisor or employer must sign the documentation verifying
that the information is true and correct. A self-employed
individual must provide the name, address and Uniform Business
Identifier of business, dates of self-employment, and a
description of specific duties. Documentation of work
experience must be provided on the appropriate departmental
form.
(iii) As proof of meeting the educational requirements,
documentation such as an official transcript or diploma; or
(b) Documentation that the applicant holds a valid
certification in the appropriate discipline issued by EPA or
by an EPA-authorized state or tribe.
(i) Applicants seeking certification based on an EPA or
EPA-authorized state or tribal certification must document to
the department that they have read and will comply with the
certification and work practice standards of these rules.
(ii) Certification based on an EPA or EPA-authorized
state or tribal certification shall be issued with an
expiration date not to exceed the date of expiration listed on
the EPA or EPA-authorized state or tribal certification.
(2) All applications for certification shall be
accompanied by:
(a) Two current passport-size photos.
(b) Applicant's name, signature and date.
(c) A check or money order made out to the department of
commerce in the amount as described in the certification fees
section of these rules.
(3) Application materials can be obtained by mail from
Department of Commerce, Lead-Based Paint Program, P.O. Box
42525, Olympia, WA 98504-2525, by phone, 360-586-5323, or
electronically at http://commerce.wa.gov/lead.
