WAC 72-171-605
Request for hearing, notice by parent. In
addition to the information required in WAC 392-172-350(2), the
parent, adult student, or the attorney representing the student
must provide notice (which must remain confidential) to the
Washington state school for the blind in a request for a hearing
to the office of the superintendent of public instruction. The
notice must include:
(1) The name of the student;
(2) The address of the residence of the student;
(3) The name of the school the student is attending;
(4) A description of the nature of the problem of the
student relating to the proposed initiation or change, including
facts relating to the problem; and
(5) A proposed resolution of the problem to the extent known
and available to the parents at the time.
